Country-specific base voltage

This PR builds on #43 by adding support for country-specific base voltages during network simplification.

Countries (including US) can define their own base voltage, while countries without an explicit override continue to use the configured default. Line types are selected according to both voltage and country, with fallback to the default line-type mapping.
