chore: relocate superpowers specs/plans under .claude/ - #27

Move docs/superpowers/{plans,specs} to .claude/{plans,specs} (docs/ is
now empty and removed). Fix the stale spec cross-references inside the
moved plan files. Document in CLAUDE.md that superpowers specs/plans must
land under .claude/ and that the Subagent-Driven execution approach is
the default. Ignore the .claude/worktrees/ directory.
