Deleted superseded patches docs (v0.7.1_PATCHES.md, v0.8.x_PATCHES.md, v0.8.x_to_v0.9.0_CHANGES.md) that were causing agents to treat pre-upgrade planning notes as current truth. Updated UPGRADE_PROCESS.md to make the cleanup rule explicit: only one patches doc should exist at a time. Signed-off-by: Jared Watts <jbw976@gmail.com>

Deleted superseded patches docs (v0.7.1_PATCHES.md, v0.8.x_PATCHES.md, v0.8.x_to_v0.9.0_CHANGES.md) that were causing agents to treat pre-upgrade planning notes as current truth. These docs can be found as historical artifacts in their respective release branches.
Updated UPGRADE_PROCESS.md to make the cleanup rule explicit: only one patches doc should exist at a time.
