Opus 4.8 fast mode is $10/$50 per MTok (cheaper than the $30/$150 of Opus 4.6/4.7). Also adds the missing claude-opus-4-7 fast_models entry, which previously fell back to standard pricing and undercounted fast Opus 4.7 usage. Standard Opus 4.8 pricing arrived via #3.
