#108 — Create `TokenIcon` component

Description:
Create apps/web/src/shared/components/TokenIcon.tsx. Maps token symbol → SVG icon. Fetches icons from a CDN or bundles common ones (BTC, ETH, XLM, USDC). Shows a generic placeholder for unknown tokens.

Acceptance criteria:

• Used in TradePanel token selector, PositionsList, and Earn pool cards
• Accessible alt text
