Summary

• Adds active-no-signal-merged-apply to promote clean active-no-signal-report rows with suggested_action=merged_to_default into cleanup_eligible lifecycle metadata.
• Revalidates containment, primary/worktree identity, protected branch names, dirty state, unpushed commits, and remote-default containment immediately before metadata writes.
• Records merge-to-default cleanup evidence including transition reason, detected timestamp, default/branch refs, heads, dirty/unpushed counts, and commits_outside_default.
