Error `stream is not readable`

Hey there, thanks for putting this awesome library together.

I am using locally and it's working well through mcp-remote. I am now trying to host this remotely via Cloudflare Workers/OpenNext.

When the client hits the /mcp endpoint, it's getting a 400, with the following error (via MCP-Inspector):

Error from MCP server: Error: Error POSTing to endpoint (HTTP 400): {"jsonrpc":"2.0","error":{"code":-32700,"message":"Parse error","data":"InternalServerError: stream is not readable"},"id":null}

Has anyone else seen this? Any ideas on how to resolve it?
