What happened?
Follow-up evidence for the SessionStart:compact reload-hook truncation report (filed 2026-08-10). New datapoint, compaction of 2026-08-11 ~05:41 PT: the hook fired and persisted its full output (65.1KB) to tool-results/hook-8538e781-*.txt, but only a ~2KB preview of PERSONAL_CLAUDE.md was injected into context. Owner-side mitigation (mandatory full disk re-read post-compaction, encoded as a doctrine card) executed successfully at 05:44 PT — full 508-line file re-read and verified. Behavior identical to the 2026-08-10 observation: hook fires, output truncated at injection. The persisted-output path works; the in-context preview cap is the gap. No regression, no improvement.
