Update P. vivax drug parameters

The current values (derived from Michael White's work) are based on either outdated information or are not yet data driven:

1. PQ and TQ efficacy parameters should be replaced with Conni's efficacy work.
2. CQ efficacy parameters should be carefully considered with regard to variation in drug resistance.
3. PQ, TQ and CQ prophylaxis parameters should be replaced with results on ongoing work with Lucy and Bamboo.
