Adding User Bug on New Postgres Instance

Summary

Issue adding new users on a new Postgres instance, getting error when creating new user on the first 3 attempts.

Steps to Reproduce

1. Start on new Postgres instance
2. Sign in as admin and attempt to add a new users
3. First 3 attempts will produce an error

Expected Behavior

Users should be added with any errors

Actual Behavior

First 3 attempts at adding new users produces an error
