## Diagnostic Protocol

2. **Test Cost** (easy to test vs. expensive/risky)
**Output:** 3-5 ranked hypotheses with rationale.
### Phase 3: Hypothesis Testing
**For each hypothesis (highest priority first):**
1. **Design Discriminating Test**
- What test would prove/disprove this hypothesis?
- What would you observe if hypothesis is TRUE?
- What would you observe if hypothesis is FALSE?
2. **Execute or Specify Test**
- If information available: evaluate immediately

### Phase 4: Root Cause Identification
**When one hypothesis remains with >0.8 confidence:**
1. **Verify Against All Symptoms**
- Does this root cause explain every observed symptom?
- Are there symptoms this doesn't explain?
2. **Check for Multiple Causes**
- Could there be more than one root cause?
- Test for interaction effects
3. **Validate Causal Path**
- Trace from root cause to each symptom
- Ensure causal chain is plausible
**Output:** Root cause with confidence level and supporting evidence.
### Phase 4b: Failure Reproduction (Mandatory)
Before attempting remediation, construct a minimal reproduction case:
1. Identify the minimal input/state that triggers the symptom
2. Confirm the reproduction case triggers the **exact same symptoms** as the original report
3. Execute reproduction in an **isolated environment** (sandbox, test suite, REPL) — never in production
4. Record `reproduction_status`:
- `confirmed` — reproduction case triggers same symptoms
- `failed` — cannot reproduce (see failure protocol below)
- `skipped` — reproduction is impractical (see skip conditions below); document why
**Do not begin remediation until `reproduction_status` is `confirmed` or `skipped` with documented rationale.** A fix applied to an unconfirmed reproduction may mask symptoms without addressing the root cause.
**Reproduction failure protocol:** If reproduction fails, drop confidence by 0.2 and return to Phase 3 with the hypothesis: *"Root cause is correct but reproduction conditions are incomplete."* Investigate environment delta, timing factors, or missing state before retrying.
**Reproduction success protocol:** Lock root cause as confirmed. Confidence floor rises to 0.85 regardless of prior evidence weight. The reproduction case becomes the **verification test** for the fix — do not discard it.
**Skip conditions:**
- Transient production event with no persistent state (document why transient)
- Purely conceptual problem (no executable code path involved)
- No executable environment available (documentation-only, design flaw)
- Requires production data that cannot be safely sampled or anonymized
