[Execution] Guarantee atomic approval invalidation when economic inputs change

Problem

Approvals can remain valid while asset metadata, issuer status, route, fee policy, or contract version changes.

Required work

Represent approval dependencies explicitly and invalidate approval atomically on any material revision.

Acceptance criteria

• Dependency versions are part of the approval digest.
• Invalidation races cannot reach submission.
• Users receive a precise reason for reapproval.
• Tests mutate every bound dependency after approval.
