Membership Application form is not working in the profile

I opened an account at erpnext.org and tried to apply for membership. It is returning an error message after clicking the 'save' button: 'Couldn't save, please check the data you have entered'.
I also have made a demo site and got the same result for both INR & USD like the main erpnext.org site:

1. USD: https://erpnext.org/memberships-usd?new=1
2. INR: https://erpnext.org/memberships?new=1
