Toolbar icons wrong size in Customize window. Entire image in View>Toolbars.

I have just added a toolbar to my product and ran into two issues:

1. When you open the Customize window, the icon size is incorrect. It looks like only the biggest size is shown. Scrolling does not work.
2. The entire sprite is shown in View > Toolbars instead of a single image.

I'll have a look to see if I can find it what causes it.
