Add pathway regions

• add "global_included" as a Boolean
• add other regions with a free text label for the region name and ISO2 country codes mapped to each
• add countries simply as ISO 2 country codes

New behavior: if user filters for country that is in a pathway region, but not listed explicitly in the pathway countries, then that pathway should still appear in the results (with the correct region showing in the geography pills)
