Add slow query logging for Prisma database calls

Log Prisma queries that exceed a configured threshold.

Acceptance Criteria:

• Prisma query execution time is measured.
• Slow queries are logged with context.
• Logs include SQL and parameters when appropriate.
• Tests verify slow query detection logic.
