Upgrade ollama ai provider package

Follow up to #470 : "the ollama-ai-provider package is no longer maintained and should be migrated to ollama-ai-provider-v2. For that, zod needs to be on version 4.0.16 or higher, which would require additional adjustments"

Yeah Ollama is still broken in latest version (9.12). More QoL improvements here: #411 (comment)

EDIT: Nevermind, I think Ollama is good now? Hard to tell what's cached and what's not. Setting the base url to http://localhost:11434/api seems to properly load models and allow them to chat...

I think the default base url is wrong though (for this app), http://127.0.0.1:11434.

Exactly, you have to use /api at the end for it to work due to the old provider — this was the workaround for now. I also changed the default placeholder, so if the default showed up without /api for you, it was probably cached from a previous installation in your browser.
